Session notes for a therapy practice.
Slide Practice does not write clinical notes and is not a health record system. What it drafts for a therapy practice is an administrative summary in four sections: what we covered, progress, next steps and next focus.
This page exists to say no clearly rather than to sell something. A therapy practice searching for note taking software is usually looking for a clinical record, and Slide Practice is not one: it is not a medical device, not electronic health record software, and not a covered entity or business associate under HIPAA. What it can do is the administrative half of the work, and the line between the two is the whole of this page.

02A finished report
Not a blank table. The document, written out.
Invented example: an administrative summary for a private practice, no clinical content, no identifying detail.

- An intake conversation about scheduling, fees, cancellation terms and the practice's own record keeping policy. No clinical material was discussed in this part of the appointment and none is recorded here.

- Administrative only: the intake form is complete, the fee agreement is signed, and the client has confirmed the preferred contact method and the two times of day that work.
- Next steps
- Send the practice policy document. Book the next four appointments. Confirm whether the client wants appointment reminders by email or by text.
- Next focus
- Review the appointment pattern after four weeks and check the fee agreement against the sliding scale band agreed at intake.

03What clients ask for
Three things the reader of this report is actually looking for.
- A clear statement of what is and is not stored, which a practice has to be able to answer for its own clients.
- Something that does not duplicate the clinical record, because two records of one appointment is a governance problem.
- A way to keep the administrative conversation out of the clinical file entirely.
04What goes wrong
Four mistakes, and each one costs a renewal rather than a mark.
- Using a general tool as a clinical record. Keep a system built for clinical records and let this hold the administrative side.
- Recording a session in which clinical material is discussed, which is not what this product is for and not what its terms permit.
- Assuming a supplier is HIPAA covered because it holds health adjacent text. Slide Practice states plainly that it is not.
- Letting an administrative summary reach a client's clinical file, where it becomes part of a record with a different retention rule.
05How it gets written

01Record the session
Record on a phone, a laptop or the call software you already work on, with everybody in the session having agreed out loud first.

02Upload the recording
Upload the file when the session ends, or paste a transcript if your call software already made one.

03Read the draft
The draft comes back in about a minute, in the four sections: What we covered, Progress, Next steps, Next focus.

04Correct it
Change anything that is wrong and add anything the recording missed. Nothing goes anywhere until you have done this.

05Send it
It goes to the client under your own name and stays on their own page with every report before it.
